Policy: Referrals to AMS will require that the primary care physician complete an Anticoagulation Enrollment Form (ACEF). The ACEF will be reviewed by the Anticoagulation Clinic staff to determine if the services can be safely managed. If further consultation is necessary to determine either the level of service or the appropriateness of the patient for anticoagulation management at the IMS, the Anticoagulation Clinic staff will discuss with the attending faculty physician. New anticoagulation patients being discharged from RR-UCLA or SM-UCLA need a referral by the primary care physician who is going to assume responsibility for follow-up care in order to participate in the AMS. The discharging physician or team may not make this referral unless he/she is the patients primary care physician. Referral Process Anticoagulation Clinic Hours (M-F 0800-1600) Primary care MD completes the ACEF The completed ACEF may be: Faxed to 310-794-6565 (preferred) E-mailed to DOMAnticoagulation@mednet.ucla.edu MD notifies the Anticoagulation Clinic of the referral during normal operating business hours (M-F 0800-1700) by calling 310-794-4054. Until the Anticoagulation Clinic is notified and a confirmation of care e-mail has been received, the care of the patient will be the responsibility of the referring MD. Referrals occurring after-hours, weekends and holidays may be e-mailed or faxed and the Anticoagulation Clinic contacted during normal operating business hours.
